Dementia Advisers are available to give the Person With Dementia and their carer, the best information, support and signposting in a way that suits the client.
This service provides high-quality information, practical advice and emotional support in the aftermath of a stroke. We begin working with a stroke survivor, their carers and their family immediately following a stroke, and will continue to provide the support people need, for as long as they need it – within their own homes and back into the community.
Sussex MSK Partnership East is the local NHS organisation that manages the care and treatment of patients with bone, joint and muscle conditions (musculoskeletal or MSK for short). Our service also provides care and support for Rheumatology patients and those with chronic pain. MSK is short for “musculoskeletal”. This term covers problems, diseases and injuries relating to muscles, bones and joints.
Patients are referred to the service by their GP or other health practitioner and are assessed by our clinical team to determine the best care pathway for their individual needs and situation.
Health in Mind is an NHS organisation providing courses and other types of therapies that help with stress, anxiety and low mood. Whatever the reasons are for feeling down, relationships, money or work, we offer a service that may help you feel positive again.
Diabetes Care for You is a service for adults living with Type 1 and Type 2 diabetes and registered with a GP practice in the Brighton and Hove and High Weald, Lewes and Havens area. This service helps people to live well with diabetes by offering advice, support and help with diabetes management and meeting personalised goals.
